I don't think its a good idea to approach her on this. Really, I'd only talk to a psychiatrist because you need to get that thought out of your friend. You said yourself, its bad, so don't try to act on it. If she's ok with it, you'll both get yourself in a REALLY bad situation. If she's not ok with you, you may never be able to mend that relationship and it will always be awkward. I'd say do what you can not to think about it.

Don't act on it. People get fantasies like this but it is very wrong. You need to control your thoughts, think of something else, meet other people. Don't approach her about it.

I would definitely reach out to someone who can help you...a psychiatrist, a hotline, someone who isn't family. Please, whatever you do, don't act on this thought. This may traumatize your sister and she may never recover or it may interfere with her adult life when it comes to men. I have a cousin who has expressed interest in me in that way and things will never be the same with me and him.
